An introduction to Advance Care Planning (ACP) – online learning event

You do not need to be dying to think about what matters most in your life. In palliative and end of life care there are many choices and decisions that influence care. Planning in advance helps personalise that care and ensure care teams understand how to support. This workshop introduces ACP as conversations over time to help people, patients and families plan.

Health & social care professionals working at all levels.

Learning Objectives:

  • To understand and feel able to apply the Universal Principles for ACP.
  • To be confident in starting and responding to an ACP conversation about what matters.
  • To know where to signpost people and find extra help & learning for yourself.
