As increasing numbers of patients access private weight-loss medications, general practice is facing important questions about safety, continuity and ongoing support.
South Somerset West PCN has developed a structured, wrap-around care model that combines clinical oversight with lifestyle-focused support to ensure patients are managed safely and consistently.
Using an Agile approach, the team is testing, learning and refining the model in real time, designing a practical, scalable and sustainable solution for general practice.
