Take advantage of funded places on our PCI-accredited training programme\, designed for frontline workers across health\, social care\, community\, and primary care settings\, including Health and Wellbeing Coaches\, Social Prescribing Link Workers\, and Care Coordinators. Working in partnership with Devon Integrated Care Board (ICB)\, we offer funded opportunities to develop skills that help people take control of their health and wellbeing by focusing on what matters most to them and empowering them to play a central role in their own care. \nCourse OverviewOur 2-day foundation course is suitable for anyone working directly to support people in a health\, wellbeing\, or care capacity\, including Primary Care Networks (PCNs)\, Integrated Neighbourhood Teams\, and the voluntary/community sector. \nThrough this course\, you will develop practical coaching and person-centred communication skills that empower people to take greater control of their health and wellbeing. Focusing on personalised care\, you will learn how to tailor your support to individual needs\, recognising wider social factors that influence wellbeing and self-management. \nYou will gain core skills that support prevention\, self-management\, shared decision-making\, and effective conversations around personalised care and support planning (PCSP)\, including identifying motivations\, managing resistance\, and adapting your approach to readiness and activation levels.
