Research and evaluation have shown that this format effectively equips attendees with the understanding and practical skills needed to apply ERI in real-world settings.  Places are limited to 20 per training workshop and attendees must be available for the full day. This training has been funded by the NHS Devon Vaccination Optimisation team and so will be free for attendees \n  \n\nMonday 19th January 2026\nMonday 26th January 2026\nMonday 23rd February 2026\n\n  \nPlease complete the Sign up form for JITSUVAX vaccine confidence training  – Fill in form  as soon as possible to secure a place\, if you would like to attend one of the training days. DESCRIPTION:Devon personalised care workshop\nTuesday 29 April from 9.30am – 12.30am at Aperture House \n\n\n\n\n\n\n\nA workshop to develop a Devon approach to a personalised care model that is integral to the components of neighbourhood health and out-of-hospital care is to take place on Tuesday 29 April from 9.30am – 12.30am at Aperture House. \n  \nWe have many tools available to us now and being developed\, such as risk stratification tools in primary care\, population health data\, health coaching and peer support\, HOPE courses\, and ORCHA digital resources. \n  \nThere is an opportunity to collaboratively agree our approach to effective and personalised care and support while working in a multi-disciplinary way. \n  \nThis workshop will complement the self-assessment and design work underway across our system to develop our neighbourhood and out of hospital models taking a personalised care approach. \n  \nThere is a small budget to support attendance of clinical colleagues. \n  \nFor further information or to express your interest to attend\, please contact Jo Curtis\, our senior locality commissioning specialist (south and west).
