Devon ICB are supporting a pilot project in relation to this module and have provided funding for 11 places on this new online programme designed for primary care. This level 6 module focuses upon the role of the multi-disciplinary team in supporting patients with physical and mental health co-morbidities, in primary care and community settings. Importance is placed on a patient centred approach, promoting involvement of the individual and their family in care planning, with a focus on common conditions including anxiety and depression, and evaluation of a range of pharmacological, non-pharmacological, and self-management strategies.
