A workshop to develop a Devon approach to a personalised care model that is integral to the components of neighbourhood health and out-of-hospital care is to take place on Tuesday 29 April from 9.30am – 12.30am at Aperture House.
We have many tools available to us now and being developed, such as risk stratification tools in primary care, population health data, health coaching and peer support, HOPE courses, and ORCHA digital resources.
There is an opportunity to collaboratively agree our approach to effective and personalised care and support while working in a multi-disciplinary way.
This workshop will complement the self-assessment and design work underway across our system to develop our neighbourhood and out of hospital models taking a personalised care approach.
There is a small budget to support attendance of clinical colleagues.
