To be achieved through these objectives:
exploration of the role of primary care professionals in advance care
planning (with particular focus on non-pharmacological aspects), including
case-based discussions to illustrate importance of communication with
patients and those close to them; and risks of bias and misperceptions about
contents of treatment escalation plan (TEP) forms.
3. Signposting to
helpful resources for professionals working in Devon.
1. Feel more
confident about initiating conversations about advance care planning; and
view this as a process rather than a one-off event.
2. Feel more
confident about completing TEP forms whose contents have been discussed with
patients and those close to them; and that convey useful clinical information

where to access reliable advice and guidance related to advance care planning
and treatment escalation.
